Bottom Carafe which has the heater & a vintage cord
Sunbeam C30A or C30B enough of the label is gone, that I don't know which model it is - I have several of both the A and B model and putting them side by side I am not able to tell the difference.
Cleaned, filled with cold water, plugged in and heated to a rolling boil.  I recently made several batches of coffee with this.  On HI it heats up properly to boil the coffee, but it does not automatically turn itself down to LO.  Once the coffee rises to the top, you can flip the switch to LO and LO works fine, but it does not happen automatically.
